Mail it, or deliver it in person, along with your check, to Deer Valley Academy at 3050 W. Agua Fria Freeway, #250, Phoenix, AZ 85027. The School will send you a receipt for your tax records.

Qualified activities may include, but are not limited to, fine arts, extracurricular field trips, community school classes, athletics, and before and after school clubs.
Parents may already be contributing. A wide variety of activities in which your student is already involved may qualify for a tax credit. The cost of extracurricular activities (up to $400) for which students must pay a fee qualifies for the tax credit.
You do not need to have a child enrolled in Deer Valley Academy for your donation to be eligible for the state tax credit.
The $400 maximum state tax credit is available for taxpayers who are filing jointly, a husband and wife who file separate returns may claim only $200 each. $200 is the maximum for a single individual or a head of household.
A state tax credit will allow you to reduce the amount of your tax liability to the state or increase your refund from the state. If you donate the maximum $400, your Arizona tax bill would be $400 less or your refund would be $400 more.
